The Salvation Army (Salvos) Youth Services encompasses a range of programs for vulnerable young people, their families, and the community. Our services have a strong focus on prevention and early intervention and enable access to safe accommodation options, Holistic case management, community outreach programs and programs that teach vital independent living skills. About the role
We are seeking an enthusiastic and experienced Case Manager to deliver high quality case management support services to young men, women, and families, including those with accompanying children who are at risk of or experiencing homelessness.
Reporting to the Client Services Coordinator, this is a permanent full-time position. This role is based in across two sites in St Kilda at the Upton Road youth refuge. At our Upton Rd Youth refuges you will be supporting young people experiencing significant crisis and homelessness in supportive & therapeutic environments. You will successfully
- Ensure ongoing assessments are made of the client’s issues, needs and circumstances from initial intake to end of support period
- Defuse and manage situations where a resident is displaying inappropriate and/or anti-social behaviour
- Identify improvement to assist in the development and implementation of processes or programs to address gaps
- Participate in a least one portfolio area team and one network strategic working group to provide expert input to influence industry or sector development
- Work to the direction of the Client Services Coordinator and Clinical practitioner on the development, implementation and review of therapeutic case plans
You will have
- Degree qualification in Social Work or related field
- Relevant experience in a Social Service environment is preferred
- A current and valid Driver’s Licence
- A Working with Children’s Check
