About the position
The Aboriginal Care Navigator, Medicare Mental Health Centre Wagga Wagga, works collaboratively with consumers to connect with services and supports in the community, facilitating person-centred and integrated care.
This position delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time. This position also works collaboratively with the other members of the Medicare Mental Health Centre Wagga Wagga team and external service providers to maximise care outcomes for Aboriginal consumers.
Medicare Mental Health Centre Wagga Wagga is a collaboration between Grand Pacific Health, Directions Health, Riverina Medical and Dental Aboriginal Corporation and Mission Australia.
This position is a permanent full-time opportunity. As a full-time position, you have the option to enjoy a rostered day off each month.
We are looking for someone with:
• Identify as an Aboriginal and/or Torres Strait Islander and be an active member within the Aboriginal communities of Wagga Wagga and surrounding areas.
• A tertiary qualification in a relevant discipline, and/or extensive experience in Aboriginal health, AOD, mental health, community services or social sciences.
• Demonstrated ability to engage with Aboriginal consumers and their families or carers to provide care navigation and referral support.
• Demonstrated understanding of supports and services available in the local and regional communities.
